Designated Gift
“I give to Mustard Seed Communities, Inc., a nonprofit corporation currently located at 200 Reservoir Street, Suite #306, Needham, MA 02494, or its successor thereto, _____________ [written amount, percentage of estate, or description of property]. This gift should be used for (state purpose) as long as Mustard Seed Communities determines that the need exists. If the stated purpose is no longer possible or prudent, Mustard Seed Communities may direct the use of my bequest to the closest related purpose.
